A broken reach-in freezer door was among violations cited in the latest food inspection reports from April 2 to April 17. A total of 11 establishments were dinged for various issues with some rendering corrections within days. A new establishment, Richmond’s Dollar Tree, received its preoperational notice. Twenty-five establishments received no violations.
Violations
- April 2: 911 Tavern, 911 E. Main St., Richmond. Six concerns. Employee observed washing hands at three-bay sink. No hand washing signage at hand sink. Open container of ready-to-eat potato salad, cottage cheese and turkey lunch meat not date marked. All corrected on site. Employee observed working with food without beard restraint. No allergens listed on menu. No consumer advisory on new menu. Correct all by April 9. FOLLOW-UP: All items corrected.
- April 7: Arby’s, 1534 N. State Road 1, Cambridge City. Seven concerns. Two employees observed working with exposed food without hair restraints. Corrected on site. No thermometer in walk-in cooler. Food debris observed on reach-in freezer, on fire extinguisher and on laminated card by hand sink. Pipe leaking in walk-in cooler; pooled water observed in plastic trays under shelves storing food. Food debris/paper debris observed on floor in walk-in cooler; under fryer; food debris observed throughout kitchen floors, walls. Hand sink next to reach-in freezer in need of repair; pulling away from wall with a gap greater than 1/4″. Correct all by April 10. Reach-in freezer in need of repair, fan broken, top hinge to door broken, top door does not seal. Correct by May 10. FOLLOW-UP: Five items corrected on April 10.
- April 7: Bismarck Donuts & Coffee, 1241 NW Fifth St., Richmond. Five concerns. A sign or poster that notifies food employees to wash their hands must be posted at all handwashing sinks used by food employees and clearly visible to food employees. Food establishment does not have written procedures available for employees to follow when responding to vomiting or diarrheal events. Correct by April 15. A written employee health policy is not maintained that details how the retail food establishment complies with restrictions and exclusions. Four in-use wiping cloths were observed lying on counters when not in use rather than in a container of sanitizer. Correct by April 10. Fourteen boxes of single-service items are stored directly on the floor rather than a minimum of 6” above the floor. Correct by April 27. FOLLOW-UP: A written employee health policy is not maintained that details how the retail food establishment complies with restrictions and exclusions. Food establishment does not have written procedures available for employees to follow when responding to vomiting or diarrheal events. Both corrected on April 17.
- April 9: Hardee’s, 1925 Chester Blvd, Richmond. One concern. Handwashing signage was not observed at employee handsinks. Correct by April 23.
- April 10: Fuelmaster, 1534 N. SR 1, Cambridge City. Two concerns. Wiping cloths were observed in soap and water instead of chemical sanitizer. Corrected on site. Three-compartment sink not functioning; no water and in need of repair. Correct by May 10.
- April 10: Western Wayne Elementary, 801 E. Delaware St., Cambridge City. Two concerns. Reach-in cooler observed at 55F; equipment in need of repair and not cooling properly. Correct by May 10. Sliced apples observed at 53F. Corrected on site.
- April 13: Heaston’s Greens Fork Family Diner, 17 E. Pearl St., Greens Fork. Two concerns. Sliced tomatoes, salad lettuce, sliced ham and sliced turkey stored in kitchen refrigerator measured above the required 41F temperature. Corrected on site. Written procedures were not provided for using time as a public health control for sausage gravy. Correct by April 23.
- April 3: Shell, 1304 Industries Road, Richmond. One concern. Drink nozzles were observed dirty and in need of cleaning. Correct by April 23.
- April 14: Bob Evans, 950 Mendelson Drive, Richmond. One concern. A secondary vacuum breaker is required at the spigot/sprayhose connection at floor sink to prevent potential backsiphonage. Correct by April 20.
- April 14: LV Pantry, 1101 S. E St., Richmond. Six concerns. No hand washing sign in employee restroom. No vomit and diarrhea policy. No employee health policy. Observed soda residue on cabinets below soda station and on front of trash can beside microwave. Microwave observed heavily soiled with food residue and in need of cleaning. Correct all by April 17. Provided education to food employee in charge due to inability to perform duties. Corrected on site. FOLLOW-UP: Items corrected.
- April 17: Way 2 Go Mart, 1240 N.W. Fifth St., Richmond. Eleven concerns. Opened packages and tubs of raw chicken were observed stored directly over boxes of potatoes in the walk-in cooler. Corrected on site. Soda nozzles at customer self-serve station are heavily soiled with a mold-like substance. Boxes of soda syrup were observed stored directly on the floor rather than a minimum of 6” above the floor. Employee clothing and personal items were observed stored directly on single service items rather than in a designated area that cannot potentially contaminate food and single-service items. Strands of dust were observed hanging from the ceiling within the food preparation areas. Boxes of single-service cups and rolls of paper towels were observed stored directly on the floor rather than a minimum of 6” above the floor. Mop was observed stored in the mops sink when not in use rather than being hung to dry. All corrected that day. The floors throughout the food preparation and storage areas are soiled with a large accumulation of dirt and debris. The walls in the food preparation and utensil washing areas are heavily soiled with an accumulation of dust and food debris. Fan guards in the walk-in beverage cooler are soiled. Vent cover over single storage area is heavily solid with dust. Correct all by April 20.

- April 17: Richmond 40 Bowl, 75 S. 37th St., Richmond. During an April 2 inspection, light cover, ceiling tiles and vent over food prep area heavily soiled with dirt/debris hanging from ceiling. Vent and ceiling tile soiled with dust/debris above microwave in need of cleaning. Dirt/dust/debris outside of pizza oven in need of cleaning. All corrected.

Have a concern?
WCHD inspects local stores, restaurants, schools and other food providers at least twice a year. Establishments have 10 days after the results of their inspection during which they can make comments or appeal before it becomes available to the public. WCHD officials consider the operator response step an opportunity to tell the public that they have corrected any violations. Any response submitted by the establishment, along with a response from health inspectors, is included with the inspection form when it becomes public. A Wayne County ordinance requires each establishment to display its previous routine inspection report adjacent to the customer entrance. If any critical violations are found, inspectors follow up to ensure the establishment has made the necessary corrections. They also make visits if members of the public submit complaints.
