Cooked pasta stored on top of a trash container, hand-washing concerns, a mold-like substance in an ice maker, a ladle wrapped with electrical tape and dozens of cold cheeseburgers were among Wayne County Health Department inspectors’ November discoveries.
Inspectors found 53 issues during recent routine inspections of local food establishments.
The classification of being in or out of compliance replaces the health department’s previous terminology of critical and noncritical violations. Compliance is based on the number of risk factor or intervention violations, and whether there are repeated issues.
WCHD conducted these assessments between Oct. 31-Nov. 24.
Great Wall violations
Inspectors identified 17 violations during a Nov. 24 assessment of Great Wall, 1390 National Road W., Richmond:
Ladle handle wrapped with electrical tape is no longer easily cleanable. Tubs of raw fish and pork were stored over containers of ready-to-eat shrimp and cooked chicken in walk-in cooler. Between use, knives are stored in the crevice between the vegetable reach-in cooler and adjacent food preparation table. Can opener is heavily soiled with food debris. Single-service utensils stored at front counter for customers to handle are stored in a way that customers can touch the portion of the utensil that is placed in consumer’s mouth. Five wiping cloths were lying on food preparation counters when not in use rather than in containers of sanitizer. A sticky fly trap was hanging over food preparation table. To be addressed that day.
An employee entered work and handled clean utensils without first washing hands. An employee chewed gum while preparing food. Paper towels weren’t provided at employee hand sink. Rice paddles are stored in containers of standing water between uses. An employee rinsed hands at three-bay sink rather than washed hands at employee hand sink. An employee handled ready-to-eat foods (peeling apples) with bare hands rather than with a suitable utensil. Completed during inspection.
Three employees prepared food without wearing hair restraints. To be addressed by Nov. 16.
Outside dumpsters are sitting on bare ground rather than on a nonabsorbent, smooth, durable material that is sloped to drain. To be addressed by May 31.
Self-closing delivery door doesn’t fully shut when closing. To be addressed by Dec. 1.
The exterior of the oven adjacent to woks and grill covers of the ventilation hood are heavily soiled with grease. To be addressed by Jan. 14.
Other violations
- Nov. 18: McDonald’s, 1725 E. Main St., Richmond. One violation. Scrapper hose at three-bay sink hangs below flood rim. An air gap of 1 inch (minimum) is required to prevent backsiphonage. To be completed by Nov. 30.
- Nov. 17: Main Street Quick Pix, 2 E. Main St., Cambridge City. Two violations. Sausage pizza was 122 degrees Fahrenheit in hot holding pizza case. Voluntarily discarded. Pizza case at 122 degrees needs repair; manager advised she’ll put in a work order. To be completed in 90 days.
- Nov. 14: Olive Garden, 4111 National Road E., Richmond. One violation. Floors in kitchen and walk-ins, and walls need cleaning. Manager advised it would be done that day.
- Nov. 14: Yamato, 4710 National Road E., Richmond. One violation. Heavily soiled grill needed cleaning. To be completed that day.
- Nov. 13: Olde Richmond Inn, 138 S. Fifth St., Richmond. Three violations. Employee handling trash and soiled dishes/utensils and loading them into the dishwasher rinsed hands at a sink in the dishwashing area rather than properly washing hands with soap at the employee hand sink before handling clean dishes/utensils. Corrected during inspection. Two boxes of boneless skinless chicken breast and a box of hickory smoked boneless ham were stored directly on the walk-in cooler’s floor. To be corrected that day. Food item that can be served undercooked (Black Angus Cheeseburger) isn’t identified with an asterisk indicating Consumer Advisory (also not asterisked) at bottom of menu. To be corrected by Dec. 19. FOLLOW-UP: On Nov. 19 visit, both items were corrected.
- Nov. 13: Positivitea, 3737 National Road E., Richmond. Four violations. Employee preparing food doesn’t have their hair properly restrained. To be corrected by Nov. 23. Paper towels weren’t provided at rear food prep area hand sink. Towels are adjacent to three-bay sink rather than at hand sink. To be corrected by Nov. 14. Stirring spoons are stored in standing water between uses. Soap wasn’t provided at rear prep area hand sink. Both to be corrected that day. FOLLOW-UP on Nov. 17: Both items corrected.
- Nov. 12: Speedway, 2414 Chester Blvd., Richmond. One violation. Floor under soda station box/syrup rack needs cleaning. Completed during inspection.
- Nov. 12: Lee’s Famous Recipe, 2410 National Road W., Richmond. One violation. West-side door has a gap at the bottom greater than one-quarter inch. To be corrected by Feb. 12.
- Nov. 12: Long John Silver’s/A&W, 3707 National Road E., Richmond. One violation. Establishment doesn’t have a certified food manager (five year). Currently, managers have certificate of achievement (three year). To be corrected by Feb. 12.
- Nov. 12: Rose Hamilton Elementary School, 1281 Round Barn Road, Richmond. One violation. Outside door needs repair because of possible pest control issues. Bottom of door was rusting.
- Nov. 10: Rose City Shell/Toasted, 1304 Industries Road, Richmond. One violation. Fountain ice machine was dirty. To be completed by Dec. 10.
- Nov. 7: La Mexicana Grill, 519 N.W. Fifth St., Richmond. Five violations. Rice prepared the previous day, stored in a plastic container in the reach-in cooler adjacent to food prep area, measured 65.5 degrees Fahrenheit rather than 41 or below. It was discarded. Hand sink in meat cutting room was inaccessible because of items stored in basin. Soap wasn’t provided at hand sink adjacent to three-bay sink. Employee cutting meat in back prep area wasn’t wearing a hair restraint. A bag of onions was stored on the floor in the back storage area. All corrected during inspection. FOLLOW-UP: Nov. 17: All items corrected.
- Nov. 6: McDonald’s, 400 Commerce Road, Richmond. One violation. No thermometers in frappe machine/cooler reach-in or egg reach-in coolers. Orders were to be made that day.
- Nov. 6: Test Intermediate School, 33 S. 22nd St., Richmond. Two violations. Cheeseburgers observed at 117.2 and 118.4 degrees Fahrenheit from serving line warmer. Sixty cheeseburgers voluntarily discarded. Serving line warmer needs repair (observed at 118 degrees; seal is broken and not closing properly). To be corrected in 90 days.
- Nov. 6: Hibberd Program Building, 900 S. L St., Richmond. One violation. Cheeseburgers just cooked in hot holding observed at 124.7 degrees Fahrenheit. Twenty-nine cheeseburgers were voluntarily discarded, although the inspector said they could have been reheated to 165 degrees to make them safe.
- Nov. 5: CVS, 2150 Chester Blvd., Richmond. One violation. Milk cooler needed cleaning. Completed during inspection.
- Nov. 4: Chili’s Grill & Bar, 4383 National Road E., Richmond. Four violations. Two employees removed gloves and put on new gloves without washing hands. Employee observed eating in food preparation area. Mops were hanging adjacent to and touching clean tubs used to store chips. All completed during inspection. Pooling water observed on floor in dry storage area from previous night’s mopping. To be completed by Dec. 4. FOLLOW-UP: Nov. 17: Item corrected.
- Nov. 3: Galo’s Italian Grill, 107 Garwood Road, Richmond. Five violations. Ice maker’s interior heavily soiled with a black mold-like substance, to be replaced by March 18, 2026. A container of in-use cooked pasta was stored on top of a trash container in the rear food prep area. A plastic tub of food was stored on top of an uncovered lettuce container in the walk-in cooler. An employee drank from a small-mouth beverage container at the main food prep area rather than an area that couldn’t possibly contaminate exposed food. A wide-mouth open beverage container was in the warewashing area. All addressed during inspection. FOLLOW-UP: Nov. 20: Item corrected.

- Nov. 6: Kroger, 3701 National Road E., Richmond. During an Oct. 16 inspection, seven expired cases of baby formula were found on shelves. Item corrected.
- Nov. 4: Carlo’s Cuisine, 1310 N.W. Fifth St., Richmond. During a Sept. 25 inspection, fruit flies were found. Item corrected.
- Oct. 31: Dollar General, 820 S. N St., Richmond. On Oct. 28, inspectors visited after receiving a complaint about mold in the milk cooler. The manager advised a professional cleaner would come the next day. On return visit, no problem was found.

- Nov. 24: JoAnn’s Cafe, 723 S. Fifth St., Richmond. No violations. Opening inspection set for Dec. 1.

About Food Establishment Inspections
Wayne County Health Department inspects local stores, restaurants, schools and other food providers at least twice per year. If any aren’t compliant, inspectors follow up to ensure the establishment has made the necessary corrections. They also make visits if members of the public submit complaints.
Establishments are required to post the most recent routine inspection adjacent to the front public entrance for potential customers to view.
