Food inspections for late April and early May found 16 establishments with violations. Citations ranged from no health or vomit/diarrhea employee policy on site to improper storage of food. Many violations were corrected on site or by their follow-up visits. An influx of temporary permits signals warmer weather as mobile food establishments frequent the area.
Violations
- April 17: Centerville Youth League, 100 Temple Drive, Centerville. One concern. Establishment did not have a vomit and diarrhea policy. Corrected on site.
- April 20: Chili’s Grill & Bar, 4383 National Road East, Richmond. One concern. An employee preparing food did not have their beard restrained. Corrected on site.
- April 20: The Leland Legacy, 900 S. A St., Richmond. One concern. Dish machine is leaking and in need of repair. Correct by July 20.
- April 20: Inn Zone, 1001 N. E St., Richmond. Two concerns. No Certified Food Manager. No hand-washing signage at hand sink or employee restroom. Correct both by April 23. FOLLOW-UP: April 23, No hand-washing signage at hand sink or employee restroom corrected. Follow up on No Certified Food Manager to be corrected by June 12.
- April 22: Arby’s, 3270 National Road E., Richmond. Two concerns. A sign or poster that notifies food employees to wash their hands was not observed in the women’s restroom. An opening greater than ¼” was observed at the bottom of the delivery door. Correct by May 7.
- April 22: Radford’s East, 81 S. 37th St., Richmond. One concern. Sanitizer bucket with cleaning cloth observed stored on floor. Corrected during inspection.
- April 22: Village Pantry, 319 E. Main St., Centerville. One concern. Hand sink did not have proper signage. Correct by April 23.
- April 23: Generations Grill & Sports Bar, 1500 N. E St., Richmond. One concern. Ice machine was observed dirty and in need of cleaning. Correct by April 23.
- April 24: By Hand and Fork, 1400 National Road W., Richmond. Five concerns. No Certified Food Manager, only Food Handler. Threshold coming off at entrance to walk-in cooler. Correct by May 26. Dish machine sanitizer concentration too low. Correct by April 28. Box of frozen sausage, box of frozen hamburger, container of diced ham, container of sausage links stored on floor in walk-in freezer, not 6 inches above floor. No hand-washing signage at hand sink next to prep cooler at kitchen/grill room. Corrected during inspection.
- April 24: Tiffin Indian-American Restaurant, 1480 N. State Road 1, Cambridge City. Three concerns. No hand-washing sign in men’s employee restroom. Manual can opener heavily soiled with food/debris. Four 23-lb bags of onions stored on floor, not 6 inches above floor. All corrected during inspection.
- April 27: Gulzar’s Indian Cuisine, 4712 National Road E, Richmond. Ten concerns. An employee was observed using their bare hands to place a garnish on top of ready-to-eat food. Box grater stored on chemical storage shelf. Employee drinks, food and personal care items observed stored on prep counter next to microwave. Corrected on site. Large pans stored directly on floor under shelving rather than a minimum of 6” above the floor. Ice scoop located at ice maker was stored on a soiled surface. Can opener located in rear food prep area is soiled with dried food debris. To correct that day. Heavy ice build-up within the walk-in freezer. Employee hand sink located in rear food prep area is no longer sealed to the adjoining wall. The mesh screening of rear delivery door is torn and in need of repair. Outdoor refuse and grease containers are stored on soil rather than on a nonabsorbent material such as concrete or asphalt that is smooth and sloped to drain. Correct all by July 27. FOLLOW-UP: April 30, items corrected.
- April 28: Galo’s, 107 Garwood Road, Richmond. One concern. Hobart hot water sanitizing dishmachine max sanitizing level did not reach 180 F. Surface temperatures of items from dishmachine maxed temperatures of 147 F rather than 160 F or above. Establishment is submerging items in three-bay sink after washing and rinsing in machine. Correct by May 13.
- April 29: La Mexicana Grill, 519 NW Fifth St., Richmond. Four concerns. Employee hand sink inaccessible due to items stored in the basin. Corrected on site. Hand soap not provided at employee hand sink. Correct by April 30. Approximately 20 packages of assorted refrigerated desserts located in the Monster reach-in cooler are not providing the following information: 1) The common name of the food or, absent a common name, an adequately descriptive identity statement. 2) If made from at least two ingredients, a list of ingredients in descending order of predominance by weight, including a declaration of artificial color or flavor and chemical perspectives if contained in the food. 3) An accurate declaration of the net quantity of contents. 4) The name and place of business of the manufacturer, packer, or distributor. 5) The name of the food source for each major food allergen contained in the food unless the food source is already part of the common or usual name of the respective ingredient. Two trays of assorted pastries, located adjacent to dry goods shelving, are not provided the labeling information listed above. Correct by May 29. FOLLOW-UP: Hand soap not provided at employee hand sink, corrected April 30.
- April 29: El Rodeo, 3611 E. Main St., Richmond. One concern. Establishment did not have vomit and diarrhea cleanup policy. Corrected on site.
- April 29: Wings & Rings, 500 Commerce Road, Richmond. One concern. Fan guards in walk-in were dirty and in need of cleaning. Correct by May 10.
- May 1: Carlo’s Cuisine, 130 NW Fifth St., Richmond. One concern. Shelving above food prep station is soiled with an accumulation of dust & debris. Correct by May 11.

Food establishment does not have written procedures available for employees to follow when responding to vomiting or diarrheal events. Both corrected.
- April 20: Bob Evans, 950 Mendelson Drive, Richmond. During an April 14 inspection, a secondary vacuum breaker is required at the spigot/sprayhose connection at floor sink to prevent potential backsiphonage. Corrected.
- April 20: Way 2 Go Mart, 1240 NW Fifth St., Richmond. During an April 17 inspection, opened package of frozen raw fish fillets were observed stored directly over boxes of potatoes in the walk-in cooler. Corrected on site.
- April 30: Richmond Village Mart, 500 W. Main St., Richmond. During a March 23 inspection, no employee health policy. No sanitizer on site or in use. No bodily fluid cleanup kit. Hand sink not accessible due to moped parked in front of hand sink along with boxes of alcohol. No thermometer in milk and egg cooler. No test strips on site or in use. Plumbing fixtures in need of repair; pipe leaking in bottom cabinet to the right of soda machine. Plumbing in need of cleaning under soda station and in bottom cabinet to right. Floors, ceilings, walls in need of cleaning. Physical facilities in want of repair; leaking roof in back storage area. No employee health policy and no vomit & diarrhea clean-up policy corrected on site. No test strips; no sanitizer; floors, walls, ceiling in need of cleaning to correct by May 6. All others corrected.

Have a concern?
WCHD inspects local stores, restaurants, schools and other food providers at least twice a year. Establishments have 10 days after the results of their inspection during which they can make comments or appeal before it becomes available to the public. WCHD officials consider the operator response step an opportunity to tell the public that they have corrected any violations. Any response submitted by the establishment, along with a response from health inspectors, is included with the inspection form when it becomes public. A Wayne County ordinance requires each establishment to display its previous routine inspection report adjacent to the customer entrance. If any critical violations are found, inspectors follow up to ensure the establishment has made the necessary corrections. They also make visits if members of the public submit complaints.
