Wayne County Health Department inspectors discovered 11 taverns, restaurants, convenience stores and a grocer out of compliance during recent routine inspections of local food establishments.
They also investigated four complaints and conducted six follow-up visits to verify that prior concerns were addressed.
The classification of being in or out of compliance replaces the previous health department terminology of critical and noncritical violations.
Compliance is based on the number of risk factor or intervention violations, and whether there are repeated issues.
WCHD conducted these assessments between Sept. 15-Oct. 3.
Compliance concerns
- Oct. 1: Legends Southside Bar, 401 S. Fifth St., Richmond. One concern. No thermometer provided.
- Sept. 29: Mack’s Place, 719 S. F St., Richmond. One concern. No employee health policy. To be corrected by Nov. 1.
- Sept. 26: Sunny View Grocery, 8421 Clyde Oler Road, Williamsburg. Two concerns. Food items made from at least two ingredients aren’t provided labels including ingredients listed in descending order. To be corrected by Jan. 1. Entrance door isn’t self-closing. To be corrected by Nov. 16, 2026.
- Sept. 26: Corner Cafe at the Leland, 900 S. A St., Richmond. Ceiling vents in storage room needed cleaning. To be corrected by Oct. 26.
- Sept. 25: Carlo’s Cuisine, 1310 N. W. Fifth St., Richmond. Two concerns. Mops aren’t being hung when drying. Twenty fruit flies observed at food preparation, warewashing and mop sink areas. Both to be corrected by Oct. 19.
- Sept. 23: Gulzar’s Indian Cuisine, 4712 National Road E., Richmond. Three concerns. At previous inspection, soap and paper towels weren’t provided at dish washing area hand sink. Both to be corrected that day. Assorted plastic containers at main prep area aren’t labeled with contents. To be corrected by Oct. 2. FOLLOW-UP: By Oct. 4 visit, items were corrected.
- Sept. 23: Little Sheba’s, 175 Fort Wayne Ave., Richmond. One concern. No employee health policy. To be corrected by Oct. 30.
- Sept. 18: Fazoli’s, 4711 National Road E., Richmond. One concern. Hot water isn’t supplied to the employee restroom hand washing sinks. FOLLOW-UP: By Oct. 3 visit, the item had been corrected.
- Sept. 18: By Hand and Fork, 1400 National Road W., Richmond. One concern. No certified food handler. To be corrected by Oct. 18.
- Sept. 17: Frisch’s Big Boy, 3704 National Road E., Richmond. One concern. Soiled microwaves observed.
- Sept. 15: Paul’s Mart LLC, 1307 Sheridan St., Richmond. One concern. Restroom self-closing door wasn’t working. To be corrected by Oct. 15.
Complaints investigated
- Oct. 2: Fountain City Food Mart, 402 U.S. 27 S., Fountain City. Complaint about Lunchables not refrigerated. Disposed of that day. Additional concern: No employee health policy.
- Sept. 30: Meijer, 2507 Chester Blvd., Richmond. Complaint about mold on fountain drink stations. No problem found.
- Sept. 22: Wings and Rings, 500 Commerce Drive, Richmond. Complaint about raw chicken served. Discussed with manager about log time. No problem found. No violations at time of inspection.
- Sept. 18: Speedway, 126 S.W. Fifth St., Richmond. Complaint about roaches. No problem found. No violations at time of inspection.

- Oct. 3: Little Caesar’s, 1503 E. Main St., Richmond. Items corrected. No violations found.
- Oct. 2: Subway, 408 E. Main St., Cambridge City. Item corrected.
- Sept. 19: Hardee’s, 2698 U.S. 35 N., Richmond. OK to reopen after fire. Discard oil in fryer. Don’t use fryer until serviced/repaired.
- Sept. 27 Pari’s Market Inc., 2267 E. Cumberland St., Dublin. Two items (no probe thermometer available and Pepsi soda nozzles needed cleaning) were corrected.

Wayne County Health Department inspects local stores, restaurants, schools and other food providers at least twice per year. If any aren’t compliant, inspectors follow up to ensure the establishment has made the necessary corrections. They also make visits if members of the public submit complaints.
Establishments are required to post the most recent routine inspection adjacent to the front public entrance for potential customers to view.
