Inspectors reported 41 items found out of compliance during routine inspections of local food establishments. 

The terminology “being in or out of compliance” replaces previous health department terminology of “critical” and “noncritical violations.” Compliance is based on the number of risk factor or intervention violations, and whether there are repeated issues.

Establishments are required to post the most recent routine inspection adjacent to the front public entrance for potential customers to view.  

Wayne County Health Department employees collected these concerns between June 26-July 18.

Not in compliance

  • June 26: 5 Arch Brewing, 129 E. Main St., Centerville. One item not in compliance: Boxes of single-use items are stored directly on the floor, rather than 6 inches or more above the floor. To be corrected by Aug. 26.
  • June 30: Catanzaro Kitchen, Eaton, Ohio. One item not in compliance: Water tank needed repair. To be corrected by July 30. Temporary permit was issued.
  • July 1: Quality Inn, 5501 National Road E., Richmond. One item not in compliance: Eggs dated “Use by June 21, 2025.” Product discarded. 
  • July 3: *Apple Dough, LLC (Little Caesars), 1503 E. Main St., Suite 100, Richmond. Two items not in compliance: Dirt/debris on south wall of storage area by walk-in cooler. Mold-like substance above three-compartment sink. Both to be corrected by Oct. 3. 
  • July 3: Dollar General No. 12902, 820 S. N St., Richmond. One item not in compliance: Reach-in cooler interiors soiled with a mold-like substance. To be corrected by Aug. 3.
  • July 7: Hampton Inn, 455 Commerce Road, Richmond. Two items not in compliance: No stem-type thermometer available at time of inspection. Ambient air thermometers in coolers were not working at time of inspection. Both to be corrected by July 17. 
  • July 7: Speedway No. 6, 325 E. Main St., Hagerstown. One item not in compliance: Stem-type, probe thermometer not provided. Corrected during inspection. 
  • July 8: Ron De Voo, 99 N. Perry St., Hagerstown. One item not in compliance: East side door has a gap greater than one-quarter inch at the bottom. To be corrected by Aug. 8.
  • July 9: Best Western Classic, 533 W. Eaton Pike, Richmond. Two items not in compliance: No test strips provided at time of inspection. To be corrected by July 18. Dirty orange juice machine requires cleaning. Corrected during inspection.
  • July 9: Phillips Family Grill, 231 N. 10th St., Richmond. Two items not in compliance: Cooked chicken not date-marked. Corrected during inspection. Dirty walk-in and reach-in coolers require cleaning. To be corrected by July 15.
  • July 10: Home2Suites, 5950 National Road E., Richmond. One item not in compliance: At Home2Market, individual packets of Pop-Tarts not marked for individual sale. To be corrected by Aug. 24.
  • July 10: Jack’s Donuts, 3380 National Road E., Richmond. Two items not in compliance: East entrance door does not close tightly on its own. A gap greater than one quarter-inch observed at the bottom of the door. Both to be corrected by August 10.
  • July 14: English Manor, 1811 S. Ninth St., Richmond. One item not in compliance: Manual can opener soiled with food debris, requiring cleaning. Corrected during inspection.
  • July 15: Fifth Street Coffee and Bagels, 211 S. Fifth St., Richmond. Four items not in compliance: Ice machine requires repair. Back door screen requires a sweep to prevent insects from entering. Dirty air vents over the grill require cleaning. All three issues to be corrected by July 25. Employee observed without hair net. Corrected during inspection.
  • July 16: El Amigo Mexican Food, 1520 N. A St., Richmond. Two items not in compliance: No paper towels at handwashing sink. To be corrected July 16.  Sink repair needed to remove gray water. Corrected during inspection.  
  • July 17: Comfort Suites, 421 Commerce Road, Richmond. Three items not in compliance: Walk-in cooler food not date-marked. No hand soap at hand-washing sink. Both to be corrected by July 27. Raw eggs placed above other foods in the walk-in cooler. Corrected during inspection.
  • July 17: Way-to-Go Mart, 1240 N.W. Fifth St., Richmond. 13 items not in compliance: 15 flies were observed throughout the food preparation and utensil washing areas. To be corrected by July 21. Spatula used at chicken fryer placed on a solid shelf below the food preparation table. Corrected during inspection. At pizza preparation table, spray bottle containing water wasn’t labeled with its contents. Corrected during inspection. Walk-in cooler fan guard soiled with a mold-like substance. To be corrected by Aug. 21. Can opener blade soiled with debris; air freshener spray bottle observed stored on a food preparation table; two containers labeled “Comet” stored over single-service items and/or clean equipment. All to be corrected by July 21. Utensils and equipment cleaned in the three-compartment sink were not rinsed after washing and before sanitizing. Corrected during inspection. Mop observed stored on a wooden box to dry, rather than being hung to dry. To be corrected by Aug. 21. Employee clothing observed placed on a box of open, single-service items rather than in a designated area for orderly storage. To be corrected Aug. 21. Sandwiches prepared by establishment and displayed in the customer reach-in cooler do not provide the following label information: 
  • Ingredients in descending order
  • Accurate declaration of net weight 
  • Manufacturer name and place of business
  • Name of the food source for each major food allergen contained in the food. 

To be corrected by July 21.

“Kool-Aid Jammers” should be stored in customer reach-in cooler and do not provide labeling information. Packaging labeling states “NOT FOR RETAIL SALE.” Sandwiches stored in customer reach-in cooler are not date-marked. Both to be corrected by July 21.

  • July 18: Taco Bell: 3029 National Road E., Richmond. One item not in compliance: No lid on employee beverage. Corrected during inspection. 
  • July 18: Wendy’s, 2324 Chester Blvd., Richmond. One item not in compliance: Employee observed not wearing a hair net. Corrected during inspection.

  • July 9: University Shell, 2320 Chester Blvd., Richmond. Unclean soda machine reported; no issues found. 

Opening inspection

  • June 23: American Legion #315, 514 S. L St., Richmond. Establishment closed for four months. OK to reopen.

Inspections

Wayne County Health Department inspects local stores, restaurants, schools and other food providers at least twice per year. If any critical violations are found, inspectors follow up to ensure the establishment has made the necessary corrections. They also make visits if members of the public submit complaints.
